CCN Value Compass

The CCN Value Compass depicts the various components of a patient persona and is a means to help care teams see their patients from more than one angle; to see the whole-person, and the etiology of each person’s current state of health and well-being.

The cardinal points on the compass represent:

  • North: functional status, risk status, and well-being
  • South: cost of the care delivered, including medical expenses, lost days of work, and other non-health care costs incurred by the patient and their family
  • East: whether the care provided matches the patient’s preferences, needs, and values, and whether the patient is satisfied with their care experience
  • West: presenting clinical problems and their manifestations
