Coordinated Care Network Whitepaper

The Accountable Care Learning Collaborative (ACLC), with a cohort of healthcare thought leaders form a variety of institutions, has clarified the core competencies a system of care must attain to successfully achieve seamless and integrated care throughout what is traditionally defined as the post-acute care space. The cohort proposes a new mental model regarding this sector, called a Coordinated Care Network (CCN). A CCN abandons the siloed treatment of patients and furthers enables coordinated and patient-centered care.

The whitepaper presents a summary of key findings from the research and discussion and introduces a series of tools designed to aid a system of care in developing their own CCN. The ACLC intends for the insights and assets shared in the paper to prompt hesitant leaders to instigate local change and provide already-committed leaders the catalyst needed to accelerate the transformation underway.
